Check Out UCHealth's Aspen Club for help with your Advanced Directive


"Aspen Club, under UCHealth, was established in 1989 to provide a wide range of program benefits including health education, screenings, hospital discounts, and social opportunities to residents ages 50-plus in Fort Collins, Loveland, Greeley, and Longmont."


They also offer free, educational classes on advanced directives (living will and medical power of attorney) and will provide free follow-up appointments after the class to help you complete the documents. Forms are available here: https://www.uchealth.org/services/palliative-care/advance-directives/. You do not need to be a member to sign up, but you must register. Classes are offered monthly and sometimes more often depending on location.
